Transition Procedures: Early Intervention (Part C) to Preschool Special Education (Part B)
When a child in Early Intervention (Part C) approaches their third birthday, a transition meeting is held to discuss next steps. At this meeting, the team considers whether the child should be referred for an evaluation to determine possible eligibility for Preschool Special Education (Part B) through Findlay City Schools. This process helps ensure that children who need continued support receive it without interruption.
The parent is a key partner throughout the transition, and their input is central to decision-making. It is important to note that not every child in Early Intervention will need or receive an evaluation at age three. An evaluation is only completed if the team suspects the child may qualify for special education services.
Step 1: Transition Planning Meeting
- The Early Intervention Service Coordinator schedules a meeting with the parent and a representative from Findlay City Schools (often the school psychologist).
- This meeting happens no later than 90 days before the child’s third birthday.
- Together, the team reviews the child’s progress and talks about possible next steps.
Step 2: Referral to the School District
- With the parents’ written consent, the child is referred to Findlay City Schools for a possible evaluation.
- This referral also occurs no later than 90 days before the child’s third birthday.
Step 3: Evaluation
- The school team, with input from the parent, decides what assessments are needed.
- Evaluations may look at areas such as communication, motor skills, social-emotional development, adaptive behavior, and early learning/pre-academic skills.
- Parents review the plan and sign consent before testing begins.
Step 4: Eligibility Meeting
- After evaluations are complete, the team meets with the parent to review the results.
- Using Ohio’s disability categories, the team determines whether the child qualifies for preschool special education services under IDEA Part B.
Step 5: Individualized Education Program (IEP)
- If the child is eligible, the IEP team (which always includes the parent) develops a written plan for services.
- The IEP must be in place on or before the child’s third birthday to ensure services continue without interruption.
